Requirements
For new students, returning students and transferees
The following required documents must be submitted to the Registrar’s Office along with a completed application form:
For Filipino Citizens
A Certificate of Good Moral Character from the Principal of the previous school
A Letter of Recommendation from either the Guidance Counselor or the Class Adviser (latest)
Original Copy of the latest report card (Form 138), with Eligibility to Transfer signed by the Principal
Photocopy of Birth Certificate and Baptismal Certificate
Two (2) pcs. 1×1 latest colored ID picture
For Foreign Students
Complete scholastic records and letter of recommendation from school last attended, with official translation from the embassy of the country they come from
Photocopy of Alien Certificate of Recognition (ACR) and passport
Original and photocopy of birth certificate
Two (2) pcs. 1×1 latest colored ID picture
Study permit
For Students with Special Needs
Copy of narrative report stating the history and diagnosis of the child
Recommendation from the previous teacher stating that the child is ready for mainstreaming or inclusion
Original and photocopy of birth certificate
Two (2) pcs. 1×1 latest colored picture
New students and transferees (including returning students) are to be given an Entrance Test for assessment. A schedule will be given by the guidance Office upon presentation of the receipt of payment. The results of the exam may be claimed from the Guidance Office on a designated date.
Applicants who pass the entrance test will be scheduled for an interview. Upon completion of the above mentioned requirements, new students/ transferees may already proceed to enrollment.
